This happens when water remains in the barrel of a fire hydrant even after it is shut off, which means that the hydrant is not draining though the weep holes. Then during cold weather the water freezes and cracks the top.

The Bonnet will then be unable to hold the necessary lubrication needed to prevent the operating nut from seizing, which can cause further damage.
